Question 1

Do Pearly Penile Papules Smell?

Accepted Answer

The straightforward answer is no. Pearly penile papules do not smell, release an odour or produce substances that cause an unpleasant smell around the penis. PPP are small, benign skin-coloured, whitish or pinkish bumps that typically develop in one or several rows around the corona, which is the rounded ridge surrounding the head of the penis. They are a recognised normal finding in male genital anatomy and are not an infection, sexually transmitted infection or consequence of inadequate personal hygiene. Source: Diagnosis and Management of Pearly Penile Papules (2016) DermNet, an established dermatology information resource, describes PPP as asymptomatic lesions that are not associated with sexual activity or a lack of hygiene. The Primary Care Dermatology Society also describes them as non-infectious and generally requiring reassurance rather than treatment. The distinction matters because men sometimes notice an unpleasant smell around the head of the penis and assume that their PPP must be responsible. This is understandable, particularly if the papules are numerous or form several closely spaced rows around the corona. However, the presence of PPP and the presence of penile odour do not necessarily have anything to do with each other.

Question 2

Why don’t PPP produce an unpleasant smell?

Accepted Answer

Pearly penile papules are small projections of normal skin tissue. They are not sweat glands, sebaceous glands or secretory structures that produce substances responsible for body odour. They do not: Produce sweat or an unpleasant-smelling secretion. Release fluid or discharge. Become infected simply because they are present. Produce an odour through sexual activity. Indicate that the penis is not being cleaned properly. Their appearance can sometimes make men more conscious of the area, particularly when the papules form a prominent ring around the glans. However, their appearance does not mean that the skin is dirty or that the papules need special cleaning. It is also important to understand that PPP are not caused by the accumulation of smegma. Smegma is a separate substance that can collect around the glans and beneath the foreskin. Its presence is not evidence that the papules are producing anything.

Question 3

Can an unpleasant smell develop around PPP?

Accepted Answer

Yes. Although PPP themselves do not smell, an unpleasant odour can develop around the same area for reasons unrelated to the papules. The corona of the glans is a natural fold or ridge of skin, and men who have a foreskin may also have a space beneath it where moisture and natural skin debris can accumulate. Several things can contribute to an unpleasant smell:

Question 4

Can smegma collect around pearly penile papules?

Accepted Answer

This is a particularly useful question because the papules are located around the corona of the glans, where natural skin folds and the surrounding anatomy may allow material to accumulate. Smegma can collect around the glans and beneath the foreskin, including in the general area where PPP are commonly found. However, it is important to distinguish between material collecting on the surrounding skin and the papules themselves producing or trapping it. PPP are not secretory structures. They do not create smegma, and their presence does not mean that a man is more likely to develop an unpleasant smell. If you notice a white or creamy substance around the glans, it may be smegma, but appearance alone cannot establish the cause. Discharge, particularly if it is new, persistent or accompanied by inflammation, should be assessed rather than automatically assumed to be smegma. Men with a foreskin may be more likely to notice smegma accumulating beneath it because the area is less exposed to the air. Gentle, regular cleaning is usually sufficient to manage normal accumulation.

Question 5

How should you clean your penis if you have PPP?

Accepted Answer

Men with PPP do not need a special hygiene routine. The papules do not require treatment, scrubbing or individual cleaning. A simple and gentle approach is generally best. Wash the penis regularly with lukewarm water. If you have a foreskin, gently retract it only as far as it comfortably moves. Rinse the glans and the surrounding area without scrubbing. Gently remove any visible smegma from the surrounding skin. Dry the area carefully before replacing the foreskin. Avoid excessive washing, which can irritate sensitive genital skin. The British Association of Dermatologists recommends gentle penile skincare and advises against fragranced products and excessive washing. Its patient guidance also explains that soaps and other cleansing products can irritate the skin of the penis. Source: BAD Patient Hub If you are prone to irritation, plain water may be sufficient. A suitable non-fragranced soap substitute can be considered where needed, particularly if recommended by a healthcare professional. Avoid using: Strong soaps or fragranced shower gels. Deodorants or perfumes on the penis. Antibacterial products without medical advice. Harsh exfoliants or abrasive cleaning materials. Household remedies intended to remove or dry out PPP. These products will not remove pearly penile papules and may make the surrounding skin uncomfortable. One further point is important for men with a tight foreskin. Never force the foreskin backwards to clean beneath it. If it cannot be retracted comfortably, or if there is persistent difficulty keeping the area clean, seek medical advice.

Question 6

When should an unpleasant penile smell be checked?

Accepted Answer

An occasional smell that improves after gentle washing may simply be associated with normal accumulation of smegma or moisture. However, a persistent or unusual smell deserves attention, particularly when accompanied by other symptoms. Consider arranging an assessment with your GP or a sexual health clinic if you notice: Redness or swelling around the glans. Persistent itching, soreness or irritation. Pain or discomfort when urinating. Thick or unusual discharge from beneath the foreskin or the urethra. Sores, blisters or broken areas of skin. Difficulty retracting the foreskin that is new or worsening. An unpleasant smell that persists despite normal, gentle hygiene. These symptoms may be associated with balanitis, an infection or another genital skin condition. The NHS recommends medical assessment when symptoms of balanitis occur, and explains that treatment depends on the underlying cause. Source: NHS If you think you may have acquired a sexually transmitted infection, a sexual health clinic can provide appropriate assessment and testing. Importantly, an unpleasant smell does not automatically mean that you have an infection. Equally, it should not automatically be attributed to PPP. The purpose of an assessment is to establish what is actually causing the smell and whether any treatment is needed.

Question 7

Can PPP be removed to get rid of penile odour?

Accepted Answer

No. Removing pearly penile papules is not a treatment for penile odour. Because PPP do not produce an unpleasant smell, their removal would not address an odour caused by smegma, balanitis, inflammation or infection. This is an important distinction for men who may be considering removal because they feel uncomfortable about the appearance of their penis or are worried that their papules are contributing to an unpleasant smell. PPP removal is a separate, elective decision. Some men choose to explore treatment because of appearance-related distress, reduced sexual confidence or the psychological impact of having prominent papules. Those concerns can be valid, but treatment should be based on an informed understanding of what removal can and cannot achieve. If an unpleasant smell is the concern, the appropriate first step is to identify its cause rather than assume that removing the papules will resolve it. The key message is simple: PPP do not smell, and removing them will not cure an unrelated cause of penile odour.

Question 8

Can PPP trap bacteria and cause a smell?

Accepted Answer

PPP do not themselves produce an odour or cause an infection. Material can accumulate around the glans or beneath the foreskin, but this is a separate issue involving the surrounding skin.

Question 9

Should I clean between each individual papule?

Accepted Answer

No. Normal gentle washing is sufficient. You do not need to scrub between the papules or use special products. Excessive cleaning can irritate the skin.

Question 10

Can a woman notice the smell of PPP during sexual intercourse?

Accepted Answer

PPP themselves do not have a smell. If an unpleasant odour is present during sexual activity, it is more likely to relate to another factor, such as normal genital secretions, smegma or an underlying skin condition.

Question 11

If I have PPP and balanitis, are they connected?

Accepted Answer

Not necessarily. PPP are benign anatomical structures, whereas balanitis is inflammation of the glans. Both can be present at the same time, but PPP do not cause balanitis.

Question 12

Will removing PPP make my penis smell better?

Accepted Answer

No. Removal does not address the causes of penile odour. If smell is your concern, it is important to establish its cause before considering any treatment for PPP.

Question 13

Do pearly penile papules smell when you sweat?

Accepted Answer

No. Sweating does not make PPP produce an odour. However, sweat and moisture around the genital area can contribute to an unpleasant smell independently of the papules.

Question 14

Can PPP smell even after washing?

Accepted Answer

No. PPP do not produce an odour, even if you have not washed recently. If an unpleasant smell persists after normal washing, consider other possible causes and seek medical advice if it continues.

Question 15

Can pearly penile papules cause smegma?

Accepted Answer

No. Smegma is a separate accumulation of material around the glans and beneath the foreskin. PPP do not produce it.

Question 16

Are pearly penile papules a sign of poor hygiene?

Accepted Answer

No. PPP are not caused by poor hygiene, inadequate washing or sexual activity. They are naturally occurring benign skin structures.

Question 17

Can penile odour mean I have an STI?

Accepted Answer

An unpleasant smell alone does not establish that you have an STI. However, some infections can cause discharge, inflammation or an unusual smell. If you have had a possible exposure or notice other symptoms, arrange appropriate sexual health testing.

Question 18

Should I use antibacterial soap to clean PPP?

Accepted Answer

No. Antibacterial soaps and strong cleansing products are unnecessary and may irritate sensitive genital skin. Gentle washing with water is usually sufficient.

Question 19

Should I have PPP removed if I am worried about penile odour?

Accepted Answer

No. Removing PPP will not resolve penile odour because the papules are not its cause. If you are considering removal for appearance-related or psychological reasons, that is a separate decision that should be made with appropriate information.
